Variety Show Signup

There are 2 parts to be completed online. After all the information is accepted, you will be directed to print out the permission form for your child to be included in the DVD. Please remember that a copy of the form with a parent's signature must be handed in for each student at the first practice. All information in Part I must be filled out completely in order to be accepted.

  • Please list the names of ALL Barclay Brook and Brookside students performing in your act. Only students attending either one of these schools are permitted. We need each child’s grade, teacher, telephone number, t-shirt size and type of refreshment donation.
  • All children will receive a t-shirt after their performance the night of the show.
  • Every act in the Variety Show needs to have one adult (“Contact Parent”) that the committee will contact to set up each mandatory rehearsal.
  • We are asking each child to donate pre-packaged goodies or individually wrapped baked goods item to sell the day of the show. This can be brought the day of the dress rehearsal. PLEASE avoid all peanut products.

If you have any questions or comments, please contact Kim Siegel by sending an email to varietyshow@bbbspta.org or ksiegel213@aol.com.

Part II Bio and Props Form

We need to know a little information about your act. The form below will help us determine how diversified our show will be.  Keep in mind this is a Variety Show and NOT a talent show.  All acts are welcome.

We also would like to know if your act is using any special props or needs our assistance with microphones, pianos, etc.

On the night of the show, each act will be introduced; in order for everyone to have their time in the spotlight, we ask each act to write a brief introduction about themselves. Please keep in mind that if the summary is too long, it will be edited by the committee.  If the following information is not available, please print this form and bring it to the first rehearsal.
